CARE MANAGER JOB DESCRIPTION

What You’ll Do
As a Care Manager, you will provide in-home support and companionship to individuals in our community while helping them live safely and comfortably in their own homes. Responsibilities may include:
- Assisting clients with Activities of Daily Living (ADLs), including:
- Bathing
- Dressing
- Grooming
- Toileting
- Mobility assistance
- Meal preparation
- Medication reminders
- Light housekeeping and laundry
- Providing companionship and emotional support
- Assisting with errands and community activities
- Maintaining accurate documentation and communication with the care team
- Creating meaningful connections with clients and families through compassionate care
Travel Requirements
This position requires travel between client homes throughout the shift. Care Managers must have:
- A valid driver’s license
- Reliable transportation
- Current auto insurance
- The ability to safely travel to multiple client locations as assigned
