Job Description

Purpose: We are looking to expand our team and are looking for a program manager. We are looking for a person who can look outside of the box for new ideas to assist individuals with disabilities to reach their fullest independence by reaching their personal goals and desires. 

Job Responsibilities

  • Monitor the Onsite staff training/orientation of new staff.
  • Facilitate Annual OSP Plan/goals implementation meetings with Individuals and the team
  • Training of staff on proper documentation using T-logs and other platforms
  • Writing and updating OSP objectives and submitting request for an Outlier to the state, as necessary.
  • Scheduling and attending Team meetings.
  • Notify the Office Manager with hiring needs for homes and participate in the termination process, as necessary.
  • Coordinating with DDPM, Families, Program managers, individuals and employees to discuss issues or concerns.
  • Conduct House Visits to assess the performance of staff.
  • Advocating for the needs of the individuals.
  • Reporting unusual incidents to the appropriate personnel (Nurse, scheduler or QA)
  • Monitor and document monthly review of client data.
  • Conduct monthly house meetings with staff.
  • Work with House Supervisor on any concerns with the individual as well as staff concerns.
  • Plan and conduct any training needs in the homes.
  • Must be able to maintain client confidentiality.
  • Must enjoy communicating and meeting new people.
  • Must be able to multitask.
  • Must be able to be flexible throughout the day.

Job Conditions

  • Position is a Full-Time position primarily M-F
  • Position will require being on an on-call rotation.
  • Position may require working direct support shifts in the evenings or weekends.
  • Position can be stressful in terms of meeting deadlines
  • Position will have both desk job elements but also direct support elements, which may involve sitting, standing and stooping and walking as well as in ordinate amount of telephone communication.
  • Minimal lifting
  • Able to communicate both verbally and in writing in English
  • Must be able utilize basic office equipment in an office setting.

Job Qualifications

  • Must have a 4 year Bachelor’s degree (Preferably in a Human Service Field area)
  • Must have 1 year of experience in working with people with a developmental or intellectual disability.
  • Must have DD certification (or have it completed within 6 months of accepting position)
