Job Description

Description: The Qualified Professional manages a case load, performs admission procedures, conducts initial and on-going assessments, develops Service Plans, performs home visits, and supervises the PCAs assigned to home care clients on their case load.

 

Qualifications:

  • The Qualified Professional is either a Registered Nurse, Licensed Social Worker, Mental Health Professional or Qualified Developmental Disabilities Specialist
  • Pass a Background Check
  • Hold and maintain required professional licensure in the state of Minnesota
  • Maintain a valid Driver’s License and automobile insurance
  • Complete the Steps for Success training within 6 months of hire

 

Job Duties:

  1. Support the mission and Code of Ethics of North Star Nursing.
  2. Serve as a role model for their respective profession.
  3. Provide general health guidance and health teaching to clients and their families at their level of understanding.
  4. Provide client care through assessment, service planning, and on-going oversight and supervision of PCA services.
  5. Observe, assess, report and document client data to maintain an accurate and timely medical record.
  6. Report any changes in client mental or physical status to their physician to prevent complications.
  7. Participate in scheduled staff meetings, in-service trainings, and client care conferences.
  8. Promote client and worker safety by enforcing current safety and infection control policies and procedures.
  9. Participate in on-going training and obtain the required number of continuing education units for their respective licensures in order to maintain active status.
  10. Provide and document necessary training, competency testing and evaluation of PCAs.
  11. Assist clients and their families in accepting responsibility for their care by promoting the use of other appropriate health resources within the home and/or community to improve overall health status.
  12. Understand the physical, emotional and developmental needs of the populations served.
  13. Communicate effectively with all types of clients.
  14. Adheres to North Star Nursing’s confidentiality policy in all situations.

 

Physical Requirements:

The following are physical requirements pertaining to the duties performed by RNs, LPNs, CNAs, PCAs, other licensed professionals and Home Health Aides assigned to home care clients. These requirements are essential functions of the job and are in addition to the necessary skills, certifications, years of experiences and/or other qualifications. Please consider the following carefully as you will be required to perform these functions safely:

You must be able to:

            1. Walk continuously

            2. Stand frequently

            3. Kneel and/or crouch on an occasional basis

            4. Forward bend and/or trunk rotation on a frequent basis

            5. Lift 8 pounds from a 10” height to a 46” eight frequently (Approximate weight of a                       food tray)

            6. Lift 18 pounds from a 20” height to a 34” height (Approximate weight of a soiled linen                  bag)

            7. Exert 40 pounds of push force frequently (Approximate force required to push an                         individual in a wheelchair)

            8. Horizontally transfer lift 60 pounds at 20” height on an occasional basis

            9. Horizontally lift boost 100 pounds at the height of 30”

            10. Grip, grasp and/or twist hands and wrists regularly
